PROCEDURES
STEP 1: Apply for Massage Technician License
at City Clerk's office.

Bring the following:
1.) An original transcript with a minimum of 300 hours
of massage training from a school approved by the Bureau
for Private Postsecondary & Vocational Education.(In
lieu of this transcript, proof of passing the National
Certification for Therapeutic Massage and Bodywork exam
may be submitted).
2.) Letter of intent of employment (i.e. J. Smith will
be employed by...name of health club, chiropractor's office,
etc. as a Massage Technician once he/she passes the Police
Permit procedure." You must work for or have a letter
of intent of employment from someone with a valid police
permit for a massage business.
3.) Fees due :On Premise Police Permit: $261
Off Premise Police Permit: $266
Fingerprinting: $27
License: $106.43
Approximate Cost : $399.43
Note: Since January 2000, these fees and regulations
may have changed. Please call your local City Clerk's
office for more information.
4.) Written proof you are at least 18 years old
5.) Three photos (1" x 1") and must have a valid Califormia
Drivers License or I.D. Card
6.) List of business and residence addresses for the
last 3 years and the inclusive dates of each such addresses.
STEP 2: Take a Massage Technician License
Examination. The test is given on the second
Thursday of each month at 8:30 a.m. at the Harbor Health
Center. When you apply for licensing, you will receive
three cards.Massage Technician examination consists of
the following:
1.)Practical (worth 60% of examination) 10 - 15 minute
swedish massage exam. Please bring someone to act as your
subject during exam. If you can not bring someone with
you, you may ask another applicant to be your partner.
You must bring your own oil/lotion to the exam. Massage
Tables, paper gowns and sheets are provided. Plan to spend
3-4 hours at the testing location on the day of the exam.
Applicants will not be allowed to take the exam without
proof of registration.
2.)Written (worth 40% of examination) 50 multiple choice
questions (25 Multiple Choice, 25 True or False), Questions
are on anatomy, physiology, bones, muscles, and general
massage techniques.
3.) Test Results A combined total score of 70% or higher
is necessary to pass. Present this card to the Los Angeles
City Clerk for your permit.

at Gaffey (End of Freeway), Continue East on Gaffey to
8th Street, Make A Left on 8th Street, Health Center Located
Between Palos Verdes Drive and Beacon Street, Parking
available in the lot
Upon recieving passing test results, the City Clerk forwards
the application to the Police Commission. An assigned
investigator will contact the applicant.
STEP 3: Medical Test: Once you
pass the written and practical test, you will need to
submit within 30 days a written statement form a state-certified
M.D. to the effect that you "have been examined and on
the basis of laboratory tests were found to be free from
all communicable and or venereal diseases" on the date
of that examination day. (Make sure you use these exact
words).
Notes: The Police Commission
only issues permits to applicants employed by a massage
or medical business in good standing with the City of
Los Angeles. To be self-employed, the massage therapist
applicant may apply for a massage business permit (on
or off premise) at this time. For information on massage
business-off premise requirements, call the Permit Processing
Section (213) 485-8081. The City Clerk's Office mails
the permit to the applicant's primary employer upon notification
of approval by the Police Commission. The Police Commission
also mails the I.D. Card to the applicant's primary employer.
(The employer is responsible for returning the I.D. Card
to the Police Commission on termination.) Massage therapists
must notify the Office of the City Clerk within 10 days
of a change of employer or residence address.
Permits expire on December 31 of the each year and must
be renewed by that date for the permittee to be eligible
for continuance in the next year. Permittees must annually
complete 6 hours of continuing education from a State
approved massage school and complete a new medical certificate
each year.
